Form 3-C
Please read the following agreement carefully before signing. This document outlines the structure, nature, and expectations of working together with Dr. Lisa M Hill.
The work provided with Dr. Lisa M Hill is faith-integrated and pastoral in nature, grounded in doctoral-level training and extensive clinical experience.
This work is not licensed psychotherapy and is not provided through state licensure.
The focus of this process is not limited to surface-level concerns or isolated situations. Instead, it centers on understanding what is shaping life from the inside out — how patterns form, how they repeat, and how they influence thoughts, responses, and relationships over time.
Spiritual life is not treated as separate from daily life. What is believed, attended to, and engaged internally will consistently influence what is experienced externally.
Sessions are structured but adaptable, guided by what is most present and what will be most helpful in the moment.
The goal of this work is not only insight, but movement — real, sustainable change that becomes evident in daily life over time.
Sessions are scheduled in advance and reserved specifically for each client.
Consistency supports meaningful progress. While the pace of the process may vary, regular engagement is an important part of the work.
This process extends beyond the session space. Awareness, reflection, and application in daily life are a natural part of how change begins to take shape.
Progress does not require perfection or complete clarity at the start. It develops through steady participation and honest engagement with what is present.
Communication between sessions may occur when appropriate and in support of the process. Sessions remain the primary space for this work.
All information shared within sessions is treated with care and respect for privacy. A full explanation of confidentiality practices, including legal limitations and privacy protections, is provided in the Informed Consent & HIPAA Disclosure.
By signing below, you acknowledge that you have read and understand the nature of services provided, the expectations of participation, and the structure of this process. You agree to engage in this work as outlined above.
